Objective: To investigate the effect of laparoscopic sacrocolpopexy in the treatment of moderate and severe uterine prolapse. Method: A total of 179 patients with moderate and severe uterine prolapse who were admitted to Zhangzhou Municipal Hospital from September 2019 to February 2022 were selected as the research objects, according to different surgical methods, they were divided into experimental group 103 cases (laparoscopic sacrocolpopexy) and control group 76 cases (laparoscopic sacral ligament shortening). The perioperative indexes, the severity of uterine prolapse, quality of life, sexual life and the changes of urinary fluidity were observed in the two groups. Result: The operative time, postoperative catheter indwelling time and postoperative hospital stay of the experimental group were shorter than those of the control group, and the intraoperative blood loss was less than that of the control group, the differences were statistically significant (P<0.05). After treatment, the total effective rate of experimental group was 94.17%, higher than 82.89% of control group (P<0.05). The quality of life scores of experimental group were lower than those of control group at 3 and 6 months after treatment (P<0.05). At 3 and 6 months after treatment, the sexual status scores of experimental group were higher than those of control group (P<0.05). After treatment, the maximum urine flow rate, average urine flow rate and maximum bladder volume in experimental group were higher than those in control group, while the residual urine volume was lower than that in control group, the differences were statistically significant (P<0.05). Conclusion: Laparoscopic sacrocolpopexy can reduce intraoperative blood loss in patients with moderate and severe uterine prolapse, shorten postoperative recovery time, improve the severity of uterine prolapse, improve their quality of life and sexual life, and improve urinary fluidity, which is worthy of clinical promotion.

Objective: To explore the effect of translateral fissure approach hematoma eva, uation on inflammatory factors, prognosis and serum granulocyte-macrophage colony-stimulating factor (GM-CSF), matrix metalloproteinase-9 (MMP-9) levels. Method: A total of 97 patients with hypertensive intracerebral hemorrhage from the Third People's Hospital of Huizhou who were treated from February 2017 to February 2022 were prospectively selected. They were divided into two groups according to the surgical method using a non randomized control method, 48 patients were treated with small bone window craniotomy hematoma clearance as the control group, and 49 patients were treated with translateral fissure approach hematoma clearance as the observation group, the results of operation were compared between the two groups. Result: The levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), C reactive protein (CRP), MMP-9, GM-CSF and overall improvement of prognosis in the observation group were better than those in the control group 7 days after surgery (P<0.05), the time of operation, the time of getting out of bed and the time of hospitalization were shorter than those of control group (P<0.05), the hematoma clearance rate and the score of ADL were higher than those of control group (P<0.05), and the amount of intraoperative blood loss and postoperative hematoma volume were lower than those of control group (P<0.05), the postoperative complication rate and NIHSS score were lower than those of control group (P<0.05). Conclusion: Neuroendoscopic transsylvian approach for hematoma evacuation in patients with hypertensive intracerebral hemorrhage can improve the hematoma clearance rate and prognosis.

Objective: To investigate the effect of Dexmedetomidine combined with Hydromorphone application in hysteroscopic surgery under laryngeal mask without muscle relaxant general anesthesia. Method: From January to December 2021, 58 patients undergoing hysteroscopic surgery in Jiangyin People's Hospital under aryngeal mask without muscle relaxant general anesthesia were selected and randomly divided into Dexmedetomidine + Hydromorphone group (DH group) and Sufentanil group (SF group), 29 patients in each group. In the DH group, general anesthesia was induced with Dexmedetomidine 0.2 μg/kg intravenously (≥10 min) combined with Hydromorphone 0.02 mg/kg and Propofol 2.5 mg/kg intravenously. In the SF group, general anesthesia was induced with Sufentanil 0.3 μg/kg and Propofol 2.5 mg/kg intravenously, Propofol 6 mg/kg intravenous pump was used for anesthesia maintenance in both groups. Heart rate (HR) and mean arterial pressure (MAP) were recorded before induction of general anesthesia (T0), before the start of surgery (T1), 10 min after the start of surgery (T2) and at the end of surgery (T3) in both groups. Bispectral index (BIS) at T1, T2 and T3 were recorded in both groups, and the time of surgery, the time of recovery from the end of surgery and the occurrence of intraoperative body movements were recorded in both groups. Result: Compared with SF group, HR and MAP of DH group were higher than those of SF group at T1 and T2, BIS at T1 and T2 were lower than those of SF group, BIS at T3 was higher than that of SF group, the incidence of intraoperative body movement was lower than that of SF group, and the recovery time was shorter than that of SF group, the differences were statistically significant (P<0.05). Conclusion: The application of Dexmedetomidine 0.2 μg/kg intravenously (≥10 min) combined with Hydromorphone 0.02 mg/kg intravenously in hysteroscopic surgery under laryngeal mask without muscle relaxant general anesthesia is reasonable and effective, resulting in smooth intraoperative anesthesia and rapid awakening.

Objective: To investigate the effect of simultaneous guided bone regeneration (GBR) with dental implantation on bone resorption around implants and periodontal status in patients with periodontitis. Method: A total of 60 patients with periodontitis treated in Ruijin People's Hospital from October 2019 to May 2021 were divided into two groups according to the random number table method, with 30 cases in each group. The control group was treated with dental implantation, and the observation group was treated with GBR during dental implantation. The patients were followed up for 12 months. The bone resorption around implants, periodontal condition, inflammatory factor level and oral health quality of life were compared between the two groups. Result: At 6 and 12 months after operation, the mesial bone absorption [(2.05±0.27), (1.25±0.22) mm] and distal bone absorption [(2.11±0.29), (1.32±0.26) mm] in the observation group were less than [(2.68±0.34), (2.29±0.32) mm], [(2.57±0.31),(2.14±0.29) mm] in the control group, with statistical differences (P<0.05). The dental probing depth (PD)[(1.62±0.21) mm], gingival index (GI) [(0.42±0.08) points], plaque index (PLI) [(0.88±0.14) points] and gingival sulcus bleeding index (BI) [(0.82±0.13) points] in the observation group 6 months after operation were lower than [(2.09±0.35) mm, (0.65±0.12) points, (1.21±0.22) points, (1.03±0.24) points] in the control group, with statistical differences (P<0.05). The level of interleukin-6 (IL-6) [(2.28±0.43) pg/mL], C reactive protein (CRP) [(4.24±0.53) mg/L] and tumor necrosis factor-α (TNF-α) [(40.31±4.28) pg/mL] 6 months after operation in the observation group were significantly lower than [(3.34±0.58) pg/mL, (6.04±1.05) mg/L, (47.32±4.87) pg/mL] of the control group (P<0.05). The scores of oral physiological function [(5.26±1.05) points], psychological function [(8.12±1.14) points], behavioral influence [(3.25±0.41) points] and pain and discomfort [(6.34±1.08) points] in the observation group 6 months after operation were lower than [(6.87±1.12), (9.86±1.23), (4.19±0.46), (7.59±1.14) points] in the control group, with statistical differences (P<0.05). Conclusion: GBR during dental implantation can enhance the therapeutic effect of periodontitis, prevent bone absorption around the implant, reduce periodontal inflammatory reaction, improve periodontal status, and improve oral health and quality of life.

Objective: To study the value of CT quantitative analysis in differentiating different pathological subtypes of pulmonary sub-solid adenocarcinoma. Method: From January 2020 to June 2022, 57 patients with sub-solid lung adenocarcinoma in Hai'an Hospital of Traditional Chinese Medicine were selected and divided into invasive adenocarcinoma cancer (IAC) and non-invasive adenocarcinoma cancer (n-IAC) according to their pathological subtypes. The clinical characteristics and CT quantitative analysis parameters of IAC patients and n-IAC patients were compared. The value of CT quantitative parameters in the differential diagnosis of pathological subtypes of sub-solid lung adenocarcinoma was analyzed by using the receiver operating characteristic (ROC) curve. Result: There were no significant differences in gender, age, nodule location, smoking history and family history between IAC patients and n-IAC patients (P>0.05); the diameter of nodules, the proportion of stage Ⅲ-Ⅳ, the proportion of poorly differentiated and the proportion of lymph node metastasis in IAC patients were all larger than those in n-IAC patients (P<0.05); the total nodule volume, solid component volume, three-dimensional average CT value, solid component volume ratio, total nodule mass, solid component mass and solid component mass ratio of IAC patients were all greater than those of n-IAC patients (P<0.05); ROC analysis showed that the cut-off value of solid component volume ratio and solid component mass ratio in differentiating pathological subtypes of lung sub-solid lung adenocarcinoma were 24.59%, 28.74%, AUC were 0.865, 0.928, sensitivity were 84.85%, 93.94%, specificity were 87.50%, 79.17%. Conclusion: CT quantitative analysis has a high value in differential diagnosis of pathological subtypes of pulmonary sub-solid adenocarcinoma.

Objective: To analyze the relationship between the expression level of miR-767-3p and epithelial-mesenchymal transition in colorectal cancer. Method: Seventy patients with colorectal cancer admitted to Jiangxi Provincial Hospital of Integrated Traditional Chinese and Western Medicine from January 2017 to December 2021 were included in the study. miR-767-3p expression and EMT indexes in colorectal cancer tissues and normal adjacent tissues were detected: the positive rate of E-cadherin, N-cadherin, β-catenin, Vimentin, and the depth of tumor invasion and lymph node metastasis, distant metastasis were examined. The correlation between miR-767-3p and other indicators was analyzed. Result: The expression level of miR-767-3p in colorectal cancer tissues was significantly higher than that in adjacent normal tissues (P<0.05). The expression levels of miR-767-3p in cancer patients with different EMT protein expressions were significantly different, and the expression levels of miR-767-3p were significantly lower in E-cadherin (+) patients, and in N-cadherin (+) patients, β-catenin (+) patients, and Vimentin (+) patients, the expression levels of miR-767-3p were significantly higher (P<0.05). The expression of EMT proteins was significantly different between the high expression group and the low expression group of miR-767-3p. The positive rate of E-cadherin in the high expression group was lower, while the positive rate of N-cadherin, β-catenin and Vimentin were higher (P<0.05). The expression level of miR-767-3p were correlated with the expression of E-cadherin, N-catenin, Vimentin, tumor differentiation and TNM stage (P<0.05), and the correlation degree was strong (E2>0.16). Conclusion: The expression level of miR-767-3p in colorectal cancer tissues is correlated with epithelial-mesenchymal transformation of tumor cells to some extent, and it is speculated that the increased expression level of miR-767-3p in cancer tissues may lead to invasion and metastasis of cancer tissues by promoting the occurrence of EMT.

Objective: To investigate the effect of modified posterolateral approach (PLA) to ankle joint combined with medial approach in the treatment of trimalleolus fracture. Method: The clinical data of 71 patients with trimalleolar fracture admitted to Jinnan Branch of Jinjiang Municipal Hospital from January 2019 to June 2021 were retrospectively analyzed, all patients received internal fixation reduction treatment, including 36 patients received traditional PLA to ankle joint combined with medial approach was traditional group and 35 patients received modified PLA to ankle joint combined with medial approach as the improved group. The efficacy, surgical and fracture healing, visual analog scale (VAS) scores, ankle function and postoperative complications were compared between the two groups. Result: There was no significant difference in efficacy between the two groups (P>0.05). The operation time and hospital stay of the improved group were shorter than those of the traditional group, and the intraoperative blood loss was less than that of the traditional group (P<0.05). There was no significant difference in fracture healing time between the two groups (P>0.05). VAS scores of the improved group were lower than those of the traditional group at 1, 2 and 3 d after surgery (P<0.05). At 3, 6 months and 1 year after surgery, ankle plantar flexion and dorsalflexion motion in the improved group were higher than those of the traditional group, and Baird-Jackson scores were higher than those of the traditional group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). Conclusion: Both modified PLA to ankle joint and traditional PLA to ankle joint combined with medial approach in the treatment of trimalleolus fracture during internal fixation reduction can achieve good curative effect, which is conducive to postoperative fracture healing, however, the advantages of the improved approach are that it can reduce intraoperative bleeding and operation time, accelerate postoperative discharge of patients, and alleviate early postoperative pain, but has no significant positive effect on reducing postoperative complications.

Objective: To investigate the effect of Sakubatril Valsartan Sodium combined with Meglumine Adenosine Cyclophosphate in the treatment of chronic heart failure and its influence on cardiac function indexes. Method: A total of 100 patients with chronic heart failure admitted to Zibo Central Hospital from January 2018 to January 2021 were selected, they were divided into single drug group and combined drug group by applying the random number table method, with 50 cases in each group. The single drug group received Meglumine Adenosine Cyclophosphate and the combined drug group received Sakubatril Valsartan Sodium combined with Meglumine Adenosine Cyclophosphate. The cardiac function indexes, neuroendocrine factor indexes, monocyte TLR4/NF-κB signaling pathway-related indexes and vascular endothelial function indexes were compared between the two groups. Result: Before treatment, there were no significant differences in left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), interventricular septal thickness (IVST), left ventricular posterior wall thickness (LVPWT) and cardiac index (CI) between the two groups (P>0.05); after treatment, CI and LVEF in both groups were higher than those before treatment, and those in combined drug group were higher than those in single drug group; LVESD, LVEDD, IVST and LVPWT were lower than those before treatment, those in combined drug group were lower than those in single drug group, the differences were statistically significant (P<0.05). Before treatment, there were no significant differences in the levels of N-terminal brain natriuretic peptide precursor (NT-proBNP), plasma renin assay (PRA) and antidiuretic hormone (ADH) between the two groups (P>0.05); after treatment, the levels of NT-proBNP, ADH and PRA in both groups were lower than those before treatment, the levels of NT-proBNP and ADH in combined drug group were lower than those in single drug group, the differences were statistically significant (P<0.05). After treatment, there was no significant difference in PRA level between the two groups (P>0.05). Before treatment, there were no significant differences in TLR4 mRNA, NF-κB mRNA, interleukin-6 (IL-6) mRNA and tumour necrosis factor-α (TNF-ɑ) mRNA levels between the two groups (P>0.05); after treatment, TLR4 mRNA, NF-κB mRNA, IL-6 mRNA and TNF-α mRNA in both groups were lower than those before treatment, those in combined drug group were lower than those in the single drug group, the differences were statistically significant (P<0.05). Before treatment, there were no significant differences in brachial artery flow mediated dilation (FMD), serum endothelin-1 (ET-1) and nitric oxide (NO) between the two groups (P>0.05); after treatment, FMD and NO levels in both groups were higher than those before treatment, those in combined drug group were higher than those in single drug group, ET-1 level were lower than those before treatment, that in combined drug group was lower than that in single drug group, the differences were statistically significant (P<0.05). Conclusion: The treatment of chronic heart failure with Sakubatril Valsartan Sodium combined with Meglumine Adenosine Cyclophosphate can improve the cardiac function and vascular endothelial function of patients, and inhibit the TLR4/NF-κB signaling pathway and neuroendocrine factor secretion of monocytes with significant effect.

Objective: To explore the efficacy of different doses of glucocorticoid combined with Propylthiouracil in the treatment of senile hyperthyroidism. Method: Sixty-six elderly patients with hyperthyroidism admitted to Gongan County People's Hospital from June 2021 to June 2022 were selected and divided into study group (33 cases) and control group (33 cases) according to random number table method. Both groups were treated with Propylthiouracil, while the control group was treated with high-dose glucocorticoid (Dexamethasone) and the study group was treated with low-dose glucocorticoid (Dexamethasone), the clinical efficacy of the two groups was compared. Result: The total effective rate and total cholesterol (TC) level in the study group were higher than those in the control group, after treatment, the incidence of adverse drug reactions, mean arterial pressure (MAP), heart rate (HR), fasting plasma glucose (FPG), adrenocorticotropic hormone (ACTH) and thyroid hormone (T4) levels were lower than those in the control group (P<0.05). Conclusion: The clinical effect of low-dose glucocorticoid combined with Propylthiouracil in the treatment of elderly hyperthyroidism is significant, which can effectively improve the relative care function and the balance of thyroid function indicators and glycolipid metabolism, and reduce the risk of adverse drug reactions, with better safety.

Objective: To study the effect of Cinepazide combined with Fasudil in the treatment of cerebral vasospasm after cerebral aneurysm embolization. Method: A total of 124 patients with cerebral vasospasm after cerebral aneurysm embolization admitted to Jiamusi Central Hospital from June 2020 to June 2022 were selected, they were divided into control group and observation group according to random number table method, with 62 cases in each group. The control group was treated with Fasudil, and the observation group was added with Cinepazide on the basis of control group. The clinical efficacy, neurological function scores, cerebral hemodynamics indexes, brain injury markers and treatment safety were compared between the two groups. Result: The total effective rate of the observation group was 91.94%, which was higher than 77.42% of the control group (P<0.05). After treatment, the NIHSS score of the observation group was lower than that of the control group, and the mini mental state examination scale (MMSE) score of the observation group was higher than that of the control group (P<0.05). After treatment, the maximum peak velocity (MPV), Vmean (Vm) and pulse index (PI) of middle cerebral artery of the observation group were higher than those of control group, and the vascular resistance index (RI) was lower than that of the control group (P<0.05). After treatment, the levels of calmodulin-dependent protein kinase 2 (CAMK2), ubiquitin carboxy-terminal hydrolase L1 (UCH-L1) and neurofibroin (NF) of the observation group were lower than those of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: The combination of Cinepazide and Fasudil has a good effect on cerebral vasospasm after cerebral aneurysm embolization, it can improve the neurological function of patients, increase the cerebral blood flow velocity, inhibit the expression level of brain injury marker protein.

Objective: To investigate the effects of long protocol in early follicular phase, antagonist protocol and short-acting long protocol in luteal phase on endometrial receptivity and clinical pregnancy in infertile patients. Method: A total of 210 female patients with infertility who received in vitro fertilization-embryo transfer (IVF-ET)/intracytoplasmic sperm injection (ICSI) for the first time in reproductive center of Affiliated Hospital of Jinggangshan University were enrolled from May 2021 to May 2022. By means of randomized control method, the patients were divided into study group 1 (n=70, long protocol in early follicular phase), study group 2 (n=70, antagonist protocol) and control group (n=70, short-acting long protocol in luteal phase). The endometrial receptivity, ovarian stimulation, embryo status and clinical pregnancy rate were compared among the three groups. Result: Before intervention, endometrial thickness (ED), endometrial volume (EV), endometrial vascular index (VI) and vascular flow index (VFI) showed no differences among the three groups (P>0.05), and the ED, EV, VI and VFI in study group 1 after intervention were higher than those in study group 2 and control group (P<0.05), and which in study group 2 were higher than those in control group (P<0.05). After intervention, the dosage and the number of use days of Recombinant Human Follitropin Alfa Solution for Injection [Gonal-F (Gn)] in study group 1 were more than those in study group 2 and control group (P<0.05), but there were no differences between study group 2 and control group (P>0.05). The level of luteinizing hormone (LH) of study group 2 was higher than those of study group 1 and control group (P<0.05), but there was no difference in LH level between study group 1 and control group (P>0.05). No differences were shown in terms of serum estradiol (E2), progesterone (P) and cycle cancellation rate (P>0.05). The effective embryo rate and high-quality embryo rate after intervention in study group 1 were higher than those in study group 2 and control group (P<0.05), but the two indicators in study group 2 and control group were no significant differences (P>0.05), and the number of transferred embryos, the number of freshly transferred cycles, whole embryo freezing and normal fertilization rate in three groups had no differences (P>0.05). The clinical pregnancy rate of study group 1 was higher than those of study group 2 and control group (P<0.05), and the clinical pregnancy rate of study group 2 and control group was no significant difference (P>0.05), and there were no differences in the number of transferred pregnancy cycles, multiple pregnancy rate, abortion rate and ectopic pregnancy rate among the three groups (P>0.05). Conclusion: Long protocol in early follicular phase can help improve endometrial receptivity, oocyte quality and clinical pregnancy in female patients with infertility, but because its down-regulation time is long, as a result, the use time of Gn is prolonged and the dosage is increased.

Objective: To investigate the effect of manual revision splinting in the treatment of distal radius fractures and the influence on wrist mobility. Method: One hundred patients with distal radius fractures treated at Nanjing County Hospital, from January 1st, 2019 to January 1st, 2022 were selected and divided into observation group (n=50) and control group (n=50) using the random number table method. The control group received surgical treatment and the observation group received manual revision splinting, and the total treatment efficiency, pain score, swelling score, wrist function score, wrist mobility, grip strength and complication rate of the two groups were compared. Result: After six months of treatment, the total effective rate of treatment in the observation group (96.00%) was higher than that in the control group (84.00%) (P<0.05). Before treatment, there were no significant differences in the visual analog scale (VAS) score, wrist swelling score and Cooney wrist function score between the two groups (P>0.05); after 1 month of treatment, the VAS score and wrist swelling score decreased in both groups, and the observation group were lower than those of the control group (P<0.05); the Cooney wrist function score increased in both groups, and the observation group was higher than that of the control group (P<0.05). Before treatment, there were no significant differences between the two groups in anterior rotation, posterior rotation, dorsiflexion, palmar flexion and radial deviation of the wrist joint (P>0.05); after 1 month of treatment, all the activities of the two groups increased, and the observation group were higher than those of the control group (P<0.05). Before treatment, the difference in grip strength on the affected side was not significant between the two groups (P>0.05); after 1 month of treatment, the grip strength on the affected side increased in both groups, and the observation group was higher than that of the control group (P<0.05). The incidence of complications in the observation group (2.00%) was lower than that in the control group (14.00%) (P<0.05). Conclusion: The treatment of distal radius fracture with manual revision splinting is effective, can reduce wrist pain, reduce swelling, improve wrist mobility and grip strength, promote wrist function recovery, reduce complications, and is safe and effective.

Objective: To investigate the effect of ultrasound-guided ultra-fine percutaneous nephrolithotomy in the treatment of lower calyceal calculus. Method: A total of 80 patients with lower calyceal calculi who were admitted to the Wuxi Second Geriatric Hospital from November 2019 to October 2022 were selected and divided into an observation group (n=40) and a control group (n=40) according to the random number table method. The control group was treated with ultrasound-guided conventional percutaneous nephrolithotomy, and the observation group was treated with ultrasound-guided ultra-fine percutaneous nephrolithotomy. The operation-related indexes, renal function indexes, inflammatory factors indexes and the incidence of complications were compared between the two groups. Result: There were no significant differences between the two groups in the operation time and the first-stage stone clearance rate (P>0.05), and the hospitalization time of observation group was shorter than that of control group, hospitalization expenses was lower than that in the control group (P<0.05). Before operation, there were no statistically significant differences in the levels of serum creatinine (Scr), blood urea nitrogen (BUN) and neutrophil gelatinase-associated lipid transport protein (NGAL) between the two groups (P>0.05); after operation, Scr, BUN and NGAL in both groups increased, but those in the observation group were lower than those in the control group (P<0.05). Before operation, there were no significant differences in the levels of tumor necrosis factor-α (TNF-α), C reactive protein (CRP) and interleukin-6 (IL-6) between the two groups (P>0.05). After operation, all the above indexes in the two groups increased, but the observation group were lower than those in the control group (P<0.05). The incidence of complications in the observation group (2.50%) was lower than that in the control group (27.50%) (P<0.05). Conclusion: Ultrasound-guided ultra-fine percutaneous nephrolithotomy is safe and effective in the treatment of lower calyceal calculi, which can reduce renal function damage, inflammatory reaction and postoperative complications.

Objective: To investigate the effect of simultaneous radiotherapy and chemotherapy of Paclitaxel for Injection in the treatment of locally advanced esophageal carcinoma in the elderly. Method: A total of 100 elderly patients with advanced local esophageal carcinoma admitted to Hai'an Hospital of Traditional Chinese Medicine from February 2019 to February 2022 were selected and divided into a combined group of 50 cases and a conventional group of 50 cases according to random number table method. The conventional group was treated with Fluorouracil (500 mg/m2) and Cisplatin Injection (20 mg/m2) + concurrent radiotherapy, and the combined group was treated with Paclitaxel for Injection (100 mg/m2) on the basis of the control group, 14 days for 1 cycle, 6 cycles. Fasting venous blood was collected before and after treatment, and squamous cell carcinoma antigen (SCCA) and cytokeratin 19 fragment (CYFRA21-1) were compared between the two groups. After 6 cycles of treatment, the objective response rate of the two groups was compared. The occurrence of adverse reactions (general fatigue, nausea and myalgia) was compared between the two groups. Result: After treatment, serum CYFRA21-1 and SCCA levels in two groups were decreased (P<0.05), and the serum CYFRA21-1 and SCCA levels in combined group were lower than those in conventional group (P<0.05). The objective remission rate in combined group (68.00%) was higher than that in conventional group (44.00%), there was statistically significant difference in objective remission rate between the two groups (P<0.05), and the incidence of adverse reactions was 14.00% in the combined group and 12.00% in the conventional group, with no statistically significant difference between the two groups (P>0.05). Conclusion: The effect of concurrent radiotherapy and chemotherapy with Paclitaxel for Injection in the treatment of esophageal cancer is relatively obvious, which can effectively improve the objective remission rate of patients, reduce the level of tumor markers, and will not significantly increase the incidence of adverse reactions in patients, so it can be popularized and applied in the elderly patients with locally advanced esophageal cancer.

Objective: To explore the clinical effect of press needle in the treatment of post-stroke superficial facial sensory disorder. Method: A total of 100 patients with post-stroke superficial facial sensory disorder in the inpatient department of Wuxi Central Rehabilitation Hospital from August 2019 to August 2022 were selected and randomly divided into a press needle group (n=50) and a control group (n=50). Both groups were given routine drug treatment, rehabilitation treatment and acupuncture treatment. The press needle group used press needle therapy for facial acupoints, and the control group used ordinary acupuncture therapy for facial acupoints. Clinical efficacy was evaluated after the treatment, and numbness visual analogue scale (VAS) and sensory function rating scale were evaluated before the treatment and at the end of the first and second courses of treatment. Result: The curative effect of press needle group was better than that of control group (P<0.05). Compared with before treatment, after the first and second course of treatment, the VAS of numbness in the two groups were decreased to different degrees, and after the second course of treatment, the press needle group was significantly better than that of the control group (P<0.05). Compared with before treatment, after the first and second course of treatment, the pain, temperature and tactile scores of the two groups were improved to different degrees, and after the second course of treatment, the press needle group were better than those of the control group in pain and tactile scores (P<0.05). Conclusion: The therapeutic effect of press needle is better to that of common acupuncture in the treatment of post-stroke superficial facial sensory disorder, in that it can alleviate the numbness symptoms, and improve the pain sensation and tactile sensation.

Objective: To observe and analyze the effect of acupuncture, massage and medical ironing method in the treatment of cervical spondylotic periarthritis of shoulder. Method: A total of 114 patients with cervical spondylotic periarthritis of shoulder admitted to Linglu Branch of Yongtai County General Hospital from May 2020 to April 2022 were selected, numbered according to the order in which the patients were treated, used random number table method as grouping tool they were randomly divided into control group and observation group, with 57 patients in each group. The control group was treated with acupuncture and massage therapy, and the observation group was treated with acupuncture and massage combined with medical ironing method. The clinical effect, visual analog scale (VAS), Constant-Murley shoulder joint function scale (CMS) scores, traditional Chinese medicine syndrome scores and inflammatory factors were compared between the two groups. Result: The total effective rate of observation group was higher than that of control group (P<0.05). After treatment, VAS score of observation group was lower than that of control group, CMS score was higher than that of control group, the differences were statistically significant (P<0.05). After treatment, the scores of joint pain, joint heaviness, joint distension and adverse flexion and extension of observation group were lower than those of control group (P<0.05). After treatment, the levels of interleukin-6 (IL-6), hypersensitive C reactive protein (hs-CRP) and tumor necrosis factor-α (TNF-α) of observation group were lower than those of control group (P<0.05). Conclusion: The combined application of acupuncture, massage and medical ironing method in patients with cervical spondylotic periarthritis of shoulder has more obvious effects, which can achieve ideal anti-inflammatory and analgesic effects, which is helpful to promote the improvement of shoulder joint function of patients, and is of great significance for improving the quality of life of patients.

Objective: To investigate the effect of TP chemotherapy combined with Xihuang Pills and press needle therapy in advanced ovarian cancer. Method: A total of 70 patients with advanced ovarian cancer admitted to Ganzhou Cancer Hospital from January 2020 to June 2022 were selected, they were randomly divided into two groups, 35 patients in each group. Both groups were treated with tumor cytoreductive surgery, the control group was treated with TP chemotherapy after surgery, and the observation group was treated with Xihuang Pills and press needle on the basis of the control group, treatment continued for 12 weeks. The clinical efficacy, tumor marker level, immune function, immunoglobulin level, quality of life and adverse reactions were compared between the two groups. Result: The total effective rate in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). The levels of carcinoembryonic antigen (CEA), carbohydrate antigen 199 (CA199) and carbohydrate antigen 153 (CA153) in the observation group were lower than those in the control group, the levels of CD3+, CD4+, CD4+/CD8+ were higher than those in the control group, and CD8+ was lower than that in the control group, the levels of IgG, IgA and IgM were higher than those in the control group, the physiological, psychological, social and environmental scores were higher than those in the control group, and the incidence of bone marrow suppression and gastrointestinal reactions were lower than those in the control group, the differences were statistically significant (P<0.05). Conclusion: TP chemotherapy combined with Xihuang Pills and press needle can improve the curative effect of patients with advanced ovarian cancer, accelerate the normalization of tumor markers, protect immune function, reduce adverse reactions of chemotherapy, and improve the quality of life.

Objective: To study the effect of evidence-based intervention of ACE-star model on the mental state of patients after modified radical mastectomy. Method: A total of 200 patients admitted to the Department of Oncology of Ningde Hospital Affiliated to Ningde Normal University from January 2018 to March 2021 were randomly divided into control group (100 cases) and research group (100 cases). The control group underwent conventional intervention, and the research group performed ACE-star model evidence-based intervention on the basis of the control group. The postoperative recovery of the upper limb function of the affected side, the incidence of postoperative complications, and the psychological state of the two groups of patients were observed. Result: The maximum angles of extension, extension, and abduction of patients in two groups were larger than those before intervention (P<0.05), and the maximum angle of extension, extension, and abduction of the research group were greater than those of the control group (P<0.05). The postoperative complication rate (5.00%) in the research group was lower than that in the control group (17.00%) (P<0.05). Objective: To explore the effect of nursing intervention based on chronic disease track mode in patients with esophageal cancer under thoracoscope. Method: A total of 120 patients with esophageal cancer who underwent thoracoscopic surgery in the Cardiothoracic Oncology Department of Zhang Ye People's Hospital Affiliated to Hexi University from January 2018 to May 2021 were selected, used the method of random numbers, all patients were divided into control group 60 cases and observation group 60 cases. The patients in the control group received routine nursing intervention, the patients in the observation group received nursing intervention based on the chronic disease track mode on the basis of the control group. The negative emotions [Hamilton depression scale (HAMD), Hamilton anxiety scale (HAMA)], self-management effectiveness [Chinese version of strategies used by people to promote health (SUPPH)], quality of life [quality of life measurement scale (EORTCQOL-C30)], disease knowledge awareness, and compliance behavior before and after treatment were compared between the two groups. The compliance behavior and postoperative complications of the two groups were compared. Result: Before intervention, there were no significant differences in HAMA scores, HAMD scores, self-management efficiency scores, quality of life scores and disease knowledge awareness scores between the two groups (P>0.05). After intervention, the scores of HAMA and HAMD in the two groups were lower than those before intervention, and the scores of self-management efficacy, quality of life and disease knowledge were higher than those before intervention (P<0.05). After intervention, the scores of HAMA and HAMD in the observation group were lower than those in the control group, and the scores of self-management efficacy, quality of life and disease knowledge were higher than those in the control group (P<0.05). After intervention, the score of compliance behavior in the observation group was higher than that in the control group (P<0.05). The incidence of postoperative complications in the observation group was lower than that in the control group (P<0.05). Conclusion: The nursing intervention based on the chronic disease track mode is applied to the population of esophageal cancer patients undergoing thoracoscopic surgery, which is beneficial to improve the patients' adverse emotional reaction, improve the patients' self-management efficiency and quality of life, increase the patients' knowledge of disease and the level of medical compliance, and reduce the incidence of postoperative complications, it is worth popularizing and applying in clinical work.

Objective: To explore the effects of acceptance and commitment therapy on psychological stress response, coping modes and psychological flexibility of intensive care unit (ICU) patients' family members. Method: A total of 70 ICU patients were selected from June 2021 to June 2022 in Nanping Second Hospital, and 70 family members who were responsible for the medical decisions of the patients were taken as the research subjects. The 70 family members were divided into the control group and the observation group according to the random number table method, with 35 cases in each group. The control group was given routine psychological care, and the observation group was given routine psychological care + acceptance and commitment therapy. The psychological stress response, coping modes and psychological flexibility were compared between the two groups. Result: After 2 weeks of nursing, the each dimensions and total scores of stress response questionnaire (SRQ) in the observation group were significantly lower than those in the control group (P<0.05). After 2 weeks of nursing, the each sub-scale and total scores of the medical coping modes questionnaire (MCMQ) in the observation group were significantly better than those in the control group (P<0.05). After 2 weeks of nursing, the scores of the acceptance and action questionnaire-second edition (AAQ-Ⅱ) in the observation group was significantly lower than that in the control group (P<0.05). Conclusion: Acceptance and commitment therapy for ICU patients' family members is helpful to reduce their psychological stress response, improve their positive coping modes and improve their psychological flexibility.

Objective: To evaluate the left ventricular function and hemodynamic characteristics of patients with hypertensive heart disease based on echocardiography. Method: One hundred and ten patients with hypertensive heart disease who were treated in Taizhou Third People's Hospital from June 2019 to March 2022 were randomly selected as the study group. At the same time, one hundred and ten healthy people who underwent echocardiography examination in author's hospital during the same period were selected as the control group, and the echocardiography test results of the two groups of subjects were recorded. According to the examination results, the left atrial function related index parameters of the two groups of subjects include left atrial diamete (LAD), left atrial diameter index (LADI), left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), related parameters of hemodynamic characteristic indexes include cardiac output (CO), cardiac output index (CI), stroke volume (SV), stroke volume index (SVI), and related parameters of left ventricular function and hemodynamic characteristics in patients with different hypertension grades were compared and analyzed. Result: Compared with the control group, the index parameters of LAD, LADI, LVESD and LVEDD in the study group were significantly increased, and the index parameters of LVEF was significantly decreased (P<0.05). Compared with the control group, the index parameters of CO, CI, SV and SVI in the study group decreased significantly (P<0.05). The LAD, LADI, LVESD and LVEDD parameters of grade Ⅲ hypertension patients were significantly higher than those of grade Ⅰ and Ⅱ, and the LVEF parameters were significantly lower than those of grade Ⅰ and Ⅱ (P<0.05). The LAD, LADI, LVESD and LVEDD parameters of grade Ⅱ hypertension patients were significantly higher than those of grade Ⅰ, and the LVEF parameters was significantly lower than those of grade Ⅰ (P<0.05). The parameters of CO, CI, SV and SVI in patients with grade Ⅲ hypertension were significantly lower than those of grade Ⅰ and Ⅱ, with statistical significances (P<0.05). The parameters of CO, CI, SV and SVI in patients with grade Ⅱ hypertension were significantly lower than those of grade Ⅰ, with statistical significances (P<0.05). Conclusion: Echocardiography can effectively evaluate the left ventricular function and hemodynamics of patients, and has certain clinical application value.

Objective: To explore the effect of staged extended individualized cardiac rehabilitation exercise program on cardiac function and quality of life in patients with chronic heart failure (CHF). Method: A total of 85 patients with CHF admitted to Jinxi People's Hospital of Kunshan City from May 2019 to May 2021 were selected, they were divided into control group and study group according to different cardiac rehabilitation intervention methods. The control group (43 cases) received routine cardiac rehabilitation exercise program intervention, and the study group (42 cases) added staged extended individualized cardiac rehabilitation exercise program on the basis of routine intervention. The improvement of cardiac function, exercise tolerance, quality of life and the incidence of re-hospitalization were compared between the two groups. Result: Before intervention, there were no significant differences in left ventricular ejection fraction (LVEF), left ventricular diameter (LVD) and left ventricular fraction shortening (LVFS) between the two groups (P>0.05); after intervention, the levels of LVEF and LVFS in both groups were higher than those before intervention, those in the study group were higher than those in the control group, LVD levels in both groups were lower than those before intervention, that in the study group was lower than that in the control group (P<0.05). Before intervention, there was no significant difference in 6 min walking distance between the two groups (P>0.05); after intervention, the 6 min walking distance in both groups were longer than those before intervention, that in the study group was higher than that in the control group (P<0.05). Before intervention, there were no statistically significant differences in Minnesota life heart failure quality questionnaire (MLHFQ) scores in all fields between the two groups (P>0.05); after intervention, MLHFQ scores in all fields in both groups were lower than those before intervention, those in the study group were lower than those in the control group (P<0.05). The incidence of readmission in the study group was lower than that in the control group (P<0.05). Conclusion: Interventions with staged extended individualized cardiac rehabilitation exercise program for patients with CHF showed significant improvements in cardiac function, exercise tolerance, and quality of life, as well as a significant reduction in the occurrence of rehospitalization, with better clinical outcomes.

Objective: To explore the diagnostic value of serum carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), carbohydrate antigen 153 (CA153) combined with human epidermal growth factor receptor 2 (HER2) level in breast cancer. Method: A total of 108 cases of breast cancer and 110 cases of breast benign tumor were selected from June 2019 to June 2021 in the Third Hospital of Nanchang and respectively set as the breast cancer group and the breast benign tumor group. In addition, 100 cases who underwent health examination in the same period were selected as the healthy control group. The serum levels of CEA, CA125, CA153 and HER2 in the three groups were detected, and the diagnostic value of the combination of four serum indicators for breast cancer was evaluated by using receiver operating characteristic curve. Result: The serum CEA, CA125, CA153, HER2 levels in the breast cancer group and the breast benign tumor group were higher than those in the healthy control group, and the serum indexes levels above the breast cancer group were higher than those in the breast benign tumor group (P<0.05). The levels of the above serum indexes in patients with breast cancer stage Ⅲ, Ⅳ were higher than those in stage Ⅰ, Ⅱ (P<0.05). The sensitivity and area under curve of combined diagnosis of serum CEA, CA125, CA153 and HER2 were higher than those of single diagnosis of breast cancer (P<0.05), and there were no significant differences in specificity compared with those of single diagnosis of each index (P>0.05). Conclusion: Serum CEA, CA125, CA153 and HER2 levels are all abnormally elevated in breast cancer, which are related to clinical stages, all of them have certain diagnostic value for breast cancer, but combined detection can improve the diagnostic value.

Objective: To explore the clinical value of maternal serum alpha fetoprotein (AFP) combined with ultrasound screening for fetal neural tube defects in the second trimester of pregnancy, so as to provide reference for clinical diagnosis and treatment of fetal neural tube defects. Method: A prospective study was conducted on pregnant women who were screened for neural tube defects in the second trimester (16-27 weeks of gestation) in Yichun Yuanzhou Modern Hospital from January 2019 to January 2022. All of them underwent ultrasound examination, and the maternal serum AFP level was detected at the same time. The screening was confirmed by autopsy after induction of labor or follow-up after birth for fetuses with neural tube abnormalities, and confirmed by telephone or outpatient follow-up for healthy fetuses. The results of induction of labor or follow-up after birth were the "gold standard", the pregnant women were divided into positive group and negative group, and the maternal serum AFP levels of the two groups were compared. The results of ultrasound examination and imaging manifestations were analyzed. At the same time, the relationship between the results of ultrasound examination and serum AFP level was analyzed, the value of maternal serum AFP combined with ultrasound in screening fetal neural tube defects in the second trimester was analyzed. Result: A total of 7 694 pregnant women were screened in this study, of which 12 were lost to follow-up. Finally, 7 682 pregnant women were included in this study. A total of 19 fetuses were diagnosed with neural tube defects, accounting for 2.47‰ (19/7 682). Ultrasound correctly diagnosed fetal neural tube malformation in 17 cases, including 7 cases of anencephaly, 1 case of open brain malformation, 1 case of encephalocele, and 8 cases of spina bifida. The maternal serum AFP level of positive group was higher than that of negative group (P<0.05). Point-biserial correlation analysis showed that the positive rate of ultrasonography was positively correlated with maternal serum AFP (r=0.532, P=0.019). The area under the curve (AUC) of maternal serum AFP combined with ultrasound in screening fetal neural tube defects in the second trimester was the largest, 0.910, and the sensitivity and specificity were 89.50% and 98.80% respectively. Conclusion: Maternal serum AFP combined with ultrasound has certain value in screening neural tube defects in the second trimester of pregnancy.

Objective: To observe the effect of Phloroglucinol on heme oxygenase-1 (HO-1), progesterone-inducible blocking factor (PIBF) levels and clinical symptoms in patients with threatened abortion. Method: A total of 84 patients with threatened abortion who were admitted to Xinyu People's Hospital from January 2020 to February 2022 were selected, they were divided into two groups according to the random number table method, with 42 cases in each group. The control group was given Progesterone treatment, on this basis, the observation group was given Phloroglucinol treatment, continuous treatment for 1 week. The therapeutic efficacy, clinical symptom improvement time, sex hormone levels, HO-1, PIBF levels and adverse reactions were compared between the two groups. Result: The total effective rate in the observation group was higher than that in the control group (P<0.05). Compared with the control group, the time of hemostasis, abdominal pain relief and contraction disappearance were shorter in the observation group (P<0.05). After treatment, human chorionic gonadotropin (β-hCG) and progesterone (P) in both groups were higher than those before treatment, and compared with the control group, observation group were higher (P<0.05). After treatment, HO-1 and PIBF in both groups were higher than those before treatment, and compared with control group, the observation group were higher (P<0.05). No serious adverse reactions occurred during the treatment. Conclusion: Phloroglucinol can effectively regulate the levels of HO-1, PIBF and sex hormones in patients with threatened abortion, promote the improvement of clinical symptoms and improve the therapeutic effect.

Objective: To explore the changes of serum matrix metalloproteinase-9 (MMP-9) concentration in patients with acute cerebral infarction in anterior and posterior circulation and its correlation with the severity of the disease. Method: A retrospective study was conducted in 88 patients with acute cerebral infarction who were admitted to the Department of Neurology, Shanghai Baoshan District Renhe Hospital from January 2013 to July 2018. There were 46 cases with anterior circulation infarction and 42 cases with posterior circulation infarction. According to the prognosis of the patients, they were divided into a death group (n=35) and a survival group (n=53). Meanwhile, 50 healthy people were selected as the control group. The level of MMP-9 was detected by ELISA, and the baseline data of patients were collected for statistical analysis. Result: The level of MMP-9 in cerebral infarction group was significantly higher than that in control group (P<0.05). The serum MMP-9 level in the anterior circulation group was significantly lower than that in the posterior circulation group, the difference was statistically significant (P<0.05). The NIHSS score in the anterior circulation group was (12.29±2.26), which was lower than (19.38±4.32) in the posterior circulation group, the difference was statistically significant (P<0.05). The NIHSS scores in the anterior circulation group and posterior circalation group were positive correlated with serum MMP-9 (r=0.651, 0.591, P<0.001). The proportion of smoking, drinking, lipid metabolism disorder, cerebral artery stenosis and posterior circulation acute cerebral infarction in the death group were significantly higher than those in the survival group, the level of MMP-9 was lower than that in the survival group, and the levels of WBC and hs-CRP were higher than those in the survival group (P<0.05). MMP-9, hs-CRP, WBC, acute cerebral infarction in posterior circulation, lipid metabolism disorder, drinking history and cerebral artery stenosis were all independent risk factors that affect the prognosis of patients (P<0.05). Conclusion: The level of MMP-9 in patients with acute cerebral infarction is significantly increased, which is positive correlated with the NIHSS scores. The level of serum MMP-9 in patients with acute cerebral infarction in posterior circulation is higher than that in patients with acute cerebral infarction in anterior circulation, and the level of MMP-9 is a prognostic factor.

Objective: To explore the influence of different midwifery methods on the labor process and complications of vaginal delivery. Method: A total of 126 cases of vaginal delivery in CR & WISCO General Hospital from June 2021 to May 2022 were selected as research objects, and were divided into control group and observation group according to different methods of midwifery, with 63 cases in each group. The control group received conventional midwifery, and the observation group received doula midwifery. The labor process, perineal outcome, maternal and infant complications and satisfaction of the two groups were recorded and compared. Result: The time of the first, second and third stage of labor and the total stage of labor in the observation group were shorter than those in the control group (P<0.05); the perineal lateral resection rate in the observation group was lower than that in the control group, the perineal integrity rate and perineal cracking degree Ⅰ were higher than those in the control group, and the perineal cracking degree Ⅱ were significantly lower than that in the control group (P<0.05); the total incidence of postpartum urinary retention, puerperal infection, postpartum hemorrhage and neonatal asphyxia in the observation group was significantly lower than that in the control group (P<0.05); the satisfaction of the observation group was significantly higher than that of the control group (P<0.05). Conclusion: Whole-course doula can effectively shorten the labor process of vaginal delivery, reduce the incidence of complications, improve maternal satisfaction, maternal outcome is good, worthy of clinical application and promotion.

Objective: To investigate the correlation between mild cognitive disorder of cerebral vascular disease and plasma asymmetric dimethylarginine (ADMA) and homocysteine (Hcy). Method: A prospective study was used in this study. A total of 120 patients diagnosed with cerebral vascular disease in Xinyu Mining Hospital from January 2017 to January 2021 were included in the study. Among them, 48 patients with cognitive disorder, all of them mild cognitive disorder, were included in the observation group, and 72 patients with normal cognition were included in the control group. The levels of ADMA and Hcy in patients were detected by enzyme-linked immunosorbent assay, and the differences between the two groups were compared, and the correlation between mild cognitive disorder and plasma ADMA and Hcy in patients with cerebral vascular disease was analyzed. Result: The MoCA score of the observation group was lower than that of the control group, and the levels of ADMA and Hcy were significantly higher than those of the control group (P<0.05). Through correlation analysis, there were negatively correlation between MoCA and ADMA, Hcy levels, and the sensitivity and accuracy of combined detection of ADMA and Hcy were significantly higher than those of single detection (P<0.05). Through ROC curve analysis, the area under the curve of joint detection was significantly higher than that of single detection (P<0.05), and the critical values of ADMA and Hcy levels in the observation group respectively was 0.68 μmol/L, 30.26 μmol/L. Conclusion: Mild cognitive disorder of cerebral vascular disease is significantly correlated with ADMA and Hcy, which can be used as an important diagnostic reference for cognitive impairment in the future.

Objective: To study the effect of the timing of labor analgesia under spinal block anesthesia on labor course and maternal and infant outcomes. Method: A total of 120 puerperae in the Obstetrics Department of the First Hospital of Nanchang from June 2021 to May 2022 were selected and randomly divided into four groups by digital table method, with 30 cases in each group. In group A and group B, intraspinal analgesia was performed when cervical dilation ≤3 cm. Group A stopped analgesia after dilation of the cervix; group B continued analgesia after dilation; group C and group D received intraspinal analgesia when the dilation of the cervix was greater than 3 cm. Group C stopped analgesia after dilation; group D continued analgesia after dilation. The duration of labor and maternal and infant outcomes were recorded in each group. Result: There were statistically significant differences in the time spent in the first stage of labor, the second stage of labor and the total stage of labor among the groups (P<0.05). There was no significant difference in the duration of the third stage of labor among the groups (P>0.05). Pairwise comparison between groups: the first stage of labor and total labor time in group A and group B were significantly longer than those in group C and group D; the second stage of labor in group A and group C were significantly shorter than those in group B and group D, the differences were statistically significant (P<0.05). There were no significant difference in the first stage of labor and total labor time between group A and group B, group C and group D (P>0.05). There were no significant differences in the duration of the second stage of labor between group A and group C, group B and group D (P>0.05). The vaginal delivery rates of group C and group D were 93.33% and 93.33%, respectively, which were higher than 70.00% and 70.00% of group A and group B. The use of Oxytocin in group C and group D was 33.33% and 26.67%, respectively, which were lower than 56.67% and 53.33% of group A and group B, the differences were statistically significant (P<0.05). There were no significant differences in vaginal delivery rate and oxytocin usage rate between group B and group A, group D and group C (P>0.05). There were no significant differences in neonatal asphyxia rate, postpartum hemorrhage volume at 2 hours, and neonatal Apgar scores at 1, 5, 10 minutes between the groups (P>0.05). There was no significant difference in the incidence of complications such as lower extremity numbness, urinary retention, pain and itching, fever and nausea among the groups (P>0.05). Conclusion: The whole process of intraspinal labor analgesia is a more appropriate analgesic method when the cervix is ..dilated >3 cm, and it can shorten the labor process, and has little effect on maternal and infant outcomes.

Objective: To explore the guiding value of bedside transcranial Doppler (TCD) ultrasound in the bridging treatment of acute cerebral infarction. Method: A total of 80 patients with acute cerebral infarction who were admitted to the Department of Neurology of the Jingdezhen No.1 People's Hospital from May 2019 to January 2022 were selected, and they were randomly divided into two groups according to random number table, with 40 cases in each group. Both groups were treated with bridging treatment. The control group was treated with reference to the Guidelines for Diagnosis and Treatment of Acute Ischemic Stroke in China during the treatment process, the observation group was additionally treated with bedside TCD to guide bridging treatment. The national institutes of health stroke scale (NIHSS) scores, modified Rankin scale (mRS) scores, and Gonner evaluated the success rate of revascularization, complication rate and mortality were compared between the two groups. Result: NIHSS score at 72 h after treatment and mRS score at 60 d after treatment in observation group were lower than those in control group (P<0.05). The vascular recirculation rate was 80.00% in the observation group after 72 h of treatment, which was higher than 57.50% in the control group (P<0.05). The complication rate in observation group was 5.00%, which was lower than 20.00% in control group (P<0.05). The fatality rate in the observation group was 10.00% after 6 months, which was lower than 27.50% in the control group (P<0.05). Conclusion: Bridging treatment can achieve good therapeutic effect in patients with acute cerebral infarction, and the bedside TCD guidance can reduce the patient's nerve defect, promote the patient's functional recovery, and help improve the success rate of revascularization during the treatment process, reduce complication rate and mortality.

Objective: To investigate the effect and clinical value of early comprehensive rehabilitation treatment in patients complicated with dysphagia after stroke. Method: A total of 120 patients complicated with dysphagia after stroke were included in the study. They were all admitted to the Department of Neurology, Shihezi People's Hospital from January 2021 to January 2022, and were randomly divided into groups using digital table extraction method. The 60 cases of control group were treated with routine rehabilitation treatment, and the 60 cases of observation group were treated with early comprehensive rehabilitation treatment. The total effective rate, swallowing function, immune function indexes, nutritional status indexes and incidence of complications were compared between the two groups. Result: The total effective rate in the observation group (96.67%) was higher than that in the control group (83.33%) (P<0.05). After treatment, the scores of water swallow test in the observation group decreased more significantly than that in the control group (P<0.05). After treatment, immunoglobulin G (IgG) and immunoglobulin M (IgM) in the observation group decreased more significantly than those in the control group (P<0.05). After treatment, the serum total protein, hemoglobin and albumin in the observation group increased more significantly than those in the control group (P<0.05). The incidence of complications in the observation group was lower than that in the control group (P<0.05). Conclusion: Patients complicated with dysphagia after stroke pay attention to early comprehensive rehabilitation treatment, which can improve the total effective rate, enhance swallowing function, improve immune level, improve the nutritional status of the body, and reduce the incidence of complications, which is of great development value.

Objective: To analyze the influencing factors of postoperative abdominal distension in patients with osteoporotic vertebral compression fractures (OVCFs). Method: The clinical data of 154 patients with OVCFs treated in Wuhan Orthopedic Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to June 2022 were retrospectively analyzed, and the risk factors of postoperative abdominal distension in patients with OVCFs were analyzed. Result: Among 154 patients with OVCFs, 33 cases (21.43%) of abdominal distension occurred after surgery. There were no significant differences in age, gender, hypertension, body mass index (BMI), smoking history, ASA classification, required fasting before surgery and postoperative eating time between the patients with or without abdominal distension (P>0.05). There were significant differences in anesthesia methods, unused patient-controlled analgesia (PCA), frequency of getting out of bed, antibiotic use and bed time between the patients with or without abdominal distension (P>0.05). logistic regression analysis showed that, general anesthesia [OR=2.880, 95%CI (1.131, 7.332)], unused PCA[OR=4.538, 95%CI (1.687, 12.213)], getting out of bed less than 3 times/d [OR=5.711, 95%CI (2.042, 15.973)], used antibiotics [OR=2.901, 95%CI (1.136, 7.403)], bed duration ≥5 d [OR=6.231, 95%CI (2.185, 17.766)] were risk factors for postoperative abdominal distension in patients with OVCFs (P<0.05). Conclusion: The incidence of postoperative abdominal distension in patients with OVCFs is high, and its occurrence is affected by the way of anesthesia, the use of PCA, the frequency of getting out of bed, the use of antibiotics, and the time in bed, clinical evidence can be used to give targeted intervention to reduce the occurrence of postoperative abdominal distension.

Objective: To study the effect of percutaneous transluminal angioplasty (PTA) under the guidance of ultrasound in the treatment of stenosis of autogenous arteriovenous fistula, and analyze the influencing factors of the treatment effect. Method: The data of patients with stenosis of autogenous arteriovenous fistula who received PTA surgery in the First Hospital of Nanchang from January 2020 to December 2021 were analyzed retrospectively. The clinical effects of PTA surgery were summarized, and the patients were divided into success group and failure group according to the therapeutic effects. The baseline data of the two groups were recorded and compared, and the related factors influencing the failure of ultrasound guided PTA in the treatment of arteriovenous fistula stenosis were analyzed by regression analysis. Result: The failure rate of PTA under ultrasound guidance was 19.05%. There were no significant differences in sex, age, body mass index, primary disease, hypercholesterolemia, location of autogenous arteriovenous internal fistula, stenosis type, internal fistula use time, original autogenous arteriovenous internal fistula operation style, time from finding stenosis to PTA, platelet, D-dimer between the failure group and the success group (P>0.05). The prevalence of hypertension, diabetes and vascular calcification in the failure group were higher than those in the success group, and the level of serum high-sensitivity C reactive protein was higher than that in the success group, with statistical significance (P<0.05). logistic regression analysis showed that hypertension, diabetes, vascular calcification and high level of high-sensitivity C reactive protein were the relevant influencing factors for the clinical effect of ultrasound-guided PTA in the treatment of arteriovenous fistula stenosis (P<0.05). Conclusion: Ultrasound guided PTA surgery for autogenous arteriovenous fistula stenosis has a high risk of failure, which is related to hypertension, diabetes, vascular calcification, and high level of high-sensitivity C reactive protein.

Objective: To study the etiological characteristics and risk factors of patients with intracranial infection complicated with pulmonary infection after craniocerebral injury. Method: The Clinical data of 236 patients with intracranial infection with craniocerebral injury admitted to the Second Affiliated Hospital of Shenyang Medical College from February 2019 to February 2022 were retrospectively analyzed, and they were divided into infected group (59 cases) and non-infected group (177 cases) according to whether they were complicated with pulmonary infection. The composition of pathogenic bacteria in the infection group was analyzed, and the baseline data of the two groups were compared. Multivariate logistic regression was used to analyze the influencing factors of intracranial infection complicated with pulmonary infection after craniocerebral injury. Result: 67 strains of pathogenic bacteria were obtained by bacterial culture from 59 patients with craniocerebral injury complicated with intracranial infection, and Gram-negative bacteria and Gram-positive bacteria were dominant. In order of proportion from high to low, they were Acinetobacter baumannii, Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus, Staphylococcus haemolyticus and Escherichia coli, respectively, the proportions were 31.34%, 22.39%, 16.42%, 13.43%, 8.96% and 7.46%, respectively. The age of the infected group was higher than that of the non-infected group, the complications were more than that of the non-infected group, and the duration of coma and hospitalization were longer than those of the non-infected group, the proportion of smoking, open injury and multiple injury of infected group were higher than those of non-infected group (P<0.05). Multivariate logistic regression analysis showed that smoking, open injury, multiple injury, older age, more complications, long coma time and long hospitalization were risk factors of craniocerebral injury with intracranial infection complicated with pulmonary infection (P<0.05). Conclusion: Gram-negative bacteria and Gram-positive bacteria are the main pathogenic bacteria in patients with intracranial infection complicated with pulmonary infection after craniocerebral injury, the risk factors include smoking, open injury, multiple injury, age, comorbidity, coma time and hospitalization.

Objective: To explore the effect of pelvic floor myoelectric stimulation biofeedback combined with Kegel training in patients with postpartum pelvic floor dysfunction. Method: A total of 94 patients with postpartum pelvic floor dysfunction who were admitted to Putian Hanjiang Hospital from February 2021 to February 2022 were selected, they were divided into two groups according to the random number table method, with 47 cases in each group. The control group was given Kegel training, and the observation group was given pelvic floor myoelectric stimulation biofeedback intervention based on Kegel training, both groups continued the intervention for 4 weeks. The pelvic floor muscle strength, stress urinary incontinence improvement and quality of life were compared between the two groups. Result: Before intervention, there was no significant difference in the latest international female sexual function index (FSFI) scores between the two groups (P>0.05); after intervention, the FSFI score of the observation group was higher than that of the control group (P<0.05). After intervention, pelvic floor strength and urinary incontinence grading of the observation group were better than those of the control group, the total effective rate of stress urinary incontinence improvement was 95.74%, which was higher than 82.98% of the control group (P<0.05). Conclusion: Pelvic floor myoelectric stimulation biofeedback combined with Kegel training intervention in postpartum patients with pelvic floor dysfunction can improve the pelvic floor muscle strength, promote the improvement of stress urinary incontinence symptoms, and improve the quality of life of patients.

Objective: To study the influencing factors of nutritional status in patients with continuous ambulatory peritoneal dialysis (CAPD). Method: The clinical data of 169 patients with CAPD admitted to the First Affiliated Hospital of Anhui Medical University from June 2019 to April 2022 were retrospectively analyzed. Nutritional risk index was used to evaluate the nutritional status of the patients, and the patients were divided into normal nutrition group and malnutrition group. The differences between the two groups were compared, and the correlation between each variable and malnutrition was analyzed, multivariate logistic regression analysis was used to analyze the results. Result: According to the evaluation results of the geriatric nutrition risk index, 83 cases (49.1%) in the normal nutrition group and 86 cases (50.9%) in the malnutrition group were obtained. There were significant differences in age, diastolic blood pressure, serum albumin, hemoglobin, bicarbonate, uric acid, C reactive protein, and transferrin saturation levels between the two groups (P<0.05). logistic regression analysis showed that age was independent risk factor for malnutrition in patients, and serum albumin, bicarbonate and uric acid were protective factors for malnutrition in patients (P<0.05). Conclusion: The incidence of malnutrition remains high in CAPD patients, age was independent risk factor for malnutrition, and serum albumin, bicarbonate and uric acid were protective factors for malnutrition.

The increasing drug resistance rate of Helicobacter pylori causes the eradication rate of empirical therapy to decrease year by year. However, due to the advantages of sensitivity, accuracy, reliability, speed and convenience of molecular-based biological antibiotic susceptibility testing technology, the tailored therapy guided by it has been increasingly recognized and valued. This article systematically reviews and comprehensively analyzes the research progress of tailored treatment effect of molecular-based antibiotic susceptibility testing. At present, molecular-based biological antibiotic susceptibility testing technology are based on polymerase chain reaction (PCR), the detection of Clarithromycin resistance mutations is mainly often used to guide the first treatment in clinic. In general, tailored therapy guided by molecular-based antibiotic susceptibility testing is superior to empirical triple therapy, but is equivalent to empirical bismuth quadruple therapy and concomitant therapy. However, when tailored treatment is performed with bismuth quadruple therapy, the efficacy is superior to empirical bismuth quadruple therapy. Although the molecular-based antibiotic susceptibility testing technology has the defects of limited coverage of antibiotic detection types, invasive sampling, and false-negative risk, the tailored treatment guided by the molecular-based antibiotic susceptibility testing technology can still improve the eradication rate of Helicobacter pylori and reduce the use of unnecessary resistant antibiotics.

The fixation of femoral neck fracture in adults is an enormous challenge in traumatic orthopedics. Although traditional internal fixation methods such as cannulated compression screw internal fixation and dynamic hip screw internal fixation have been widely used in clinical practice and have achieved specific efficacy, however, the incidence of postoperative complications such as fracture nonunion and femoral head necrosis is still high. Therefore, some new types of internal fixation have emerged, such as the femoral neck system and dynamic compression locking system. In this paper, the results of clinical and biomechanical studies about the new internal fixation modalities will be compared with the previous internal fixation modalities to provide some references in selecting treatment modalities for adult patients with femoral neck fractures.
