Abstract Objective： To study the treatment of intramedullary nail fixed femoral shaft middle fracture effects.Method：145 cases of internal fixation failure situation bone graft were selected， The needle method， expanding intramedullary or not and fixed way were used by depending on the patient's specific conditions.Result： The operatio of different nail ways， the average blood loss and infection chance compared， the difference was statistically significant （P<0.05）；in the expansion of pulp or not， the average blood loss and fat embolism comparison difference was statistically significant（P<0.05）；fixed way without influence the fracture healing， and there was no statistically significant difference （P>0.05）. Conclusion： For midway through femoral fractures，especially comminuted fracture with intramedullary nail fixation surgery procedures is now more classic surgical internal fixation method.
